Toronto Chronic Diseases Centre (TCDC) is an excellent healthcare professional centre. It is consist of a group of 20 excellent and experienced pain specialist, internist,  allergist and clinical immunologist, respiratologist, hematologist, cardiologist, endocrinologist, rheumatologist, gastroenterologist, nephrologist, EMG specialist, orthopedic surgeon, general surgeon, urology surgeon, and nurse educators etc. The centre provides free medical services for patients who have OHIP health cards. The medical centre not only focuses on treatment of common chronic diseases, such as diabetes, chronic pains, allergy, arthritis, autoimmune diseases, osteoporosis, COPD/asthma, blood diseases, hypertension, digestive diseases, peripheral nerve disorders (EMG), heart diseases, kidney diseases, orthopedic surgery, general surgery and urology surgery etc., but also provides comprehensive chronic diseases self-management education programs, and follow-ups provided by our experienced specialists with one-stop medical services in English, Mandarin, Cantonese, etc. All services are covered by OHIP, and free of charge for patients.

The clinic’s features are concentrating on patient-centered care, providing professional medical services, and trying our best to take care of and assist patients to prevent diseases, relieve pains, prevention and treatment of their diseases and complications. The centre helps patients to effectively use medical resources, improve the level of disease control, knowledge and self-management ability, reduce patients’ waiting time for seeing specialists, provide maximum convenience for patients, improve the efficiency of diagnosis and treatment of diseases, and optimize the quality of medical care services.

  • A native of Saskatoon Saskatchewan, Dr K. Lee Choi came to Toronto for medical school and earned his MD degree in 1978. He completed residencies in Internal Medicine and Dermatology and research fellowships in Basic Immunology. He has practiced General Dermatology in the Scarborough community since 1987. Dr. Lee Choi has over 35 years experience in common skin diseases in dermatology.

Specialist 17: Ms. L. Fan, Registered Nurse (RN), Certified Diabetes Educator (CDE), Ph.D
Ms. L. Fan is a RN, CDE, Ph.D, has over 30 years experience in clinical nursing management, patients education and research in China and Canada. She has been working as a RN, CDE nearly 20 years in Toronto focusing on diabetes self-management education and research. She has already provided individual diabetes self-management education for thousands of patients with diabetes in the Greater Toronto Areas (GTA) and she is well received by patients and doctors.  She won “Public Services and Education” award from the Canadian Diabetes Association in 2014, and she also won the “Diabetes Educator Award” in 2014 and 2015 from the Canadian Society of Endocrinology and Metabolism.  In 2016, she won "Distinguished Volunteer Service Award" from Canadian Diabetes Association. She obtained her bachelor degree and master degree from School of Nursing, Beijing Union Medical College in Beijing China, and obtained her Ph.D from School of Nursing, University of Toronto, Canada. She worked at Chinese PLA General Hospital as a Chief Nurse, Professor, Head Nurse, and supervisor of postgraduate students in the Department of Endocrinology.
  She was a Senior Clinical Nurse Specialist in clinical nursing, management and patient education in diabetes and other endocrine diseases. Dr. Fan has actively engaged in diabetes self-management education and research for more than 30 years, and published more than 80 papers (among them, more than 50 papers are as the first author) on diabetes education and complications prevention research in China, Canada and USA. She won Grade II and III awards of Science and Technology Advance Awards of Chinese PLA, and won twice Grade I Awards of Science and Technology Advance Award from Beijing Nursing Association, and received over 20 scientific research awards from Chinese Diabetes association, Chinese Nursing Association, Canada Diabetes association, and University of Toronto, etc.

Free Group Chronic Diseases Self-Management Education Programs
Ms L. Fan, RN, CDE, Ph.D is providing free  group chronic diseases self-management education classes  in Mandarin for people with chronic diseases at Toronto Chronic Diseases Centre. (Sessions are not avaliable now).

2.    Or you can come to our clinic front desk to get a blank referral form, or you can also go to our website www.TCDCclinic.ca to download the form and bring it to your family physician and signed by him/her, and fax to 416-335-1719 or email it to us: room210.frontdesk@gmail.com.

3.    When we receive your referral form from your family physician, our reception will contact with you to confirm your appointment date and time.

4.    Please bring your valid health card and all the current medications you are taking when you sees the specialist.

5.    Please arrive 10 - 15 minutes earlier than the appointment time for registration.
